29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a. On or about February 11, 2022, an employee was not provided a safe means of access to a Ladder Jack scaffold, exposing the employee to a 24-foot fall. b. On or about February 11, 2022, employees were not provided a safe means of access to a Ladder Jack scaffold, exposing the employees to a 15-foot fall. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about February 11, 2022, an employee was not protected from a fall while working from a Ladder Jack scaffold, exposing the employee to a 24-foot fall. b. On or about February 11, 2022, employees were not protected from a fall while working from a Ladder Jack scaffold, exposing the employees to a 15-foot fall. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
