29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made. Aisle(s) or passageway(s) were not kept in good repair. Permanent aisles or passageways were not appropriately marked: (a) On or about February 08, 2022 - Stack-off/shipping department, employees were exposed to struck-by hazards when the aisles/passageways were not marked where powered industrial trucks were operated. (b) On or about February 08, 2022 - Stack-off/shipping department, employees were exposed to struck-by hazards when aisles/passageways were damaged/uneven where powered industrial trucks were operated. (c) On or about February 08, 2022 - Stack-off/shipping department, employees were exposed to struck-by hazards when there was not sufficient safe clearance for the forklift operators/pallet jack operators to transport product without driving over damaged floor locations/uneven grates/drains or near pedestrians.
