At the commercial roofing job site, at 6901 SW 24th Avenue, Gainesville, Florida, on the roof: On or about February 10, 2022, the employer exposed four roofing employees to eye injury hazards in that the employer did not ensure that each employees using pneumatic nailer guns and other debris-producing tools wore safety glasses to protect from flying debris.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a. At the commercial roofing job site, at 6901 SW 24th Avenue, Gainesville, Florida, on the 3:12 pitch roof: On or about February 10, 2022, the employer exposed four roofing employees installing a new roofing system to 14-foot to 40-foot fall hazards from the edges of the lower roof in that they were not protected by a fall protection system. Perry Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard , 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1274527, Repeat Serious citation number 1, item number 1 and was affirmed as a final order on January 31, 2018, with respect to a workplace located at 121 SE 121 SE 16th Avenue, Building J, Gainesville, Florida.
