29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Paint Line Department - On or about January 13, 2022, an employee was exposed to machine hazards associated with rotating parts when wiping down and cleaning a roller on the Bridle 3 Paint Line Oven equipment, job #04-2705, serial #2705, during normal operations, without alternative protective measures. The employee suffered a severe arm injury. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to protect the employee from machine servicing hazards.     b) Paint Line Department - On or about January 20, 2022, employees on the second shift were exposed to machine hazards associated with rotating parts when performing a "complete" cleaning of several rollers on the Bridle 3 Paint Line Oven equipment, job #04-2705, serial #2705.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to protect the employee from machine servicing hazards.
