29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about February 9, 2022, employees were exposed to a fall hazard of approximately 12 feet while performing construction activities including accessing the roof to repair a leak in the roof and suitable fall protection was not used.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface to which the ladder was used to gain access: On or about February 9, 2022, employees were exposed to a fall hazard of approximately 12 feet while ascending and descending an extension ladder leaned up against the side of the building because the side rails did not extend at least three feet above the landing surface.
