29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Docks 9 & 10, OSI Group, 4201 S. Ashland Avenue, Chicago, IL 60609 - On or about February 3, 2022, the employer did not ensure that employees who operate powered industrial trucks, including the Toyota 8FBCU25 forklift truck, were properly trained, evaluated and certified to perform tasks in a safe manner, thereby exposing employees to struck-by hazards. b) Sheet Metal Shop, Key West Metal Industries, Inc., 13831 Kostner Avenue, Crestwood, IL 60445 - On or about February 14, 2022, the employer did not ensure that employees who operate powered industrial trucks, including the Nissan MCP1F2A25LV forklift truck, were properly trained, evaluated and certified to perform tasks in a safe manner, thereby exposing employees to struck-by hazards.
