29 CFR  1926.652(g)(1)(iii):Employees were not protected from the hazard of cave-ins when entering or exiting the area protected by shields:  a) J & H Reinforcing & Structural Erectors, Inc. located at or near Oh-850 And Jackson Pike Gallipolis, OH 45631:  On and preceding February 15, 2022, the employer did not ensure that employees entering a trench shield using a ramp were protected from the hazard of cave-ins.   A ramp used for entry into the trench shield had unprotected vertical faces of the trench varying from 5-9 feet along both sides of the ramp, exposing employees to caught-in/struck-by hazards.

29 CFR  1926.652(g)(2): Excavations of earth material to a level no greater than 2 feet (.61m) below the bottom of a shield was permitted:   a) J & H Reinforcing & Structural Erectors, Inc. located at or near Oh-850 And Jackson Pike Gallipolis, OH 45631:  On and preceding February 15, 2022, the employer did not ensure that earth material below the bottom of the trench shield was limited to no more than 2 feet.  The trench shield being used for protection had three to four feet of dirt below the bottom of the shield while an employee was working in it thereby exposing the employee to caught-in/struck-by hazards.
