29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks. On or about February 14, 2022, the operator of a powered industrial truck (PIT) left the controls of the PIT unattended while employees worked from the elevated work platform. This exposed the employees to a fall hazard greater than six(6) feet to the ground below.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel. On or about February 14, 2022, employees were working from a work platform elevated by a powered industrial truck (PIT) and the work platform was not securely attached to the lift carriage. This exposed the employees to a fall hazard.
