29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns. On or about February 8, 2022 at this workplace, an employee attached metal wall exterior panels to a building under construction from an elevated platform and did not wear a protective helmet to avoid possible head injuries.

29 CFR 1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On or about February 8, 2022 at this workplace, employees attached metal wall exterior panels to a building under construction. The employees were exposed to fall hazards while working from an elevated Telehandler Personnel Work Platform at the height greater than 10 feet from a lower level. Instance: A) EE#1 personal fall arrest lanyard was not attached to the boom or basket. B) EE#2 was not wearing a body belt with lanyard or personal fall arrest harness.
