Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On or about 2/15/2022, A subcontractor for MAPP, LLC was installing masonry building materials on a wall of a commercial building located at 215 S. Kingshighway in Cape Girardeau, MO. MAPP, LLC, the controlling employer, did not conduct an adequate inspection of the job site, materials, and equipment.  The subcontractor owner and five employees were installing masonry materials while working from scaffolding approximately 15 feet above the ground. They were exposed to the following hazards:  a)	The subcontractor owner and five employees were working from a scaffold platform approximately 15 feet above the ground without utilizing fall protection.  b)	The subcontractor owner and five employees did not use a ladder to access the scaffolding platform.  The owner and the employees were observed accessing the scaffolding platform by climbing up and down the scaffolding.  c)	The owner of the subcontractor company was operating a SkyTrak Telehandler, without proper training and certification, to transport material to his employees exposing them to struck-by hazards.  d)	The subcontractor owner and five employees were working on scaffolding, which was not fully planked. The platform units were not installed so that the space between adjacent units was no more than 1 inch wide.
