29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) Building Exterior: An employee was exposed to an 12 foot fall hazard while applying cement to lay block from a tubular welded frame scaffold with missing planks. Scaffold was not fully decked or planked between front and uprights, on or about 5/20/22

29 CFR 1926.1153(c)(2)(ii): Water was not applied at flow rates sufficient to minimize release of visible dust for tasks performed using wet methods:  a) Building elevator shaft: The employer did not apply water to minimize the release of visible dust while dry cutting a concrete, the employee was exposed to silica dust while dry cutting concrete block with a hand held demo saw inside the elevator shaft, on or about 5/20/22.
