On or about February 16, 2022, employees were exposed to struck-by hazards while operating a Hyundai forklift SN: FU0310796 which had a brake light that did not work. 2. On or about February 16, 2022, employees were exposed to struck-by hazards while operating a Hyster Forklift SN: G005A11520W which had a worn parking brake. 3. On or about February 16, 2022, employees were exposed to struck-by hazards while operating a Hyundai forklift, SN: 02*12201 which had a worn parking brake, a strobe light and brake light that did not work. 4. On or about February 16, 2022, employees were exposed to struck-by hazards while operating a Hyster forklift, SN: F2 which had a brake light and strobe light that did not work.
