29 CFR  1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  On or about February 17, 2022, the employer did not ensure employees are able to open the exit route door from the inside at all times without keys, tools, or special knowledge. Employees working in the facility were exposed to fire hazards from the locks being used on the emergency exit doors in the following instances:  A) The exit door located on the west wall in the "production" area of the facility had a cylinder lock keyed from the outside, and a thumb turn knob on the inside, requiring special knowledge to open in the event of an emergency.   B) The exit door located on the east wall  in the "warehouse" area of the facility has a keyed and locked doorknob that allows employees to open door from both sides, and also has two sliding security bolts that locked the door from the inside, requiring special knowledge to open in the event of an emergency.  C) The exit door located in the northwest corner of the "warehouse" area of the facility was affixed with four barrel bolts that kept the door locked from the inside, requiring special knowledge to open in the event of an emergency.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route.   On or about February 17, 2022, the employer did not ensure that all exit routes were kept free of obstructions. Cardboard boxing, a wheeled staircase, and 55 gallon drum, were placed within exit routes in the Production and Warehouse areas, thereby exposing employees to fire and emergency egress hazards.  A) The exit route located on the west wall in the "production" area of the facility was obstructed by stacks of cardboard boxes.  B) The exit route located on the east wall  in the "warehouse" area of the facility was obstructed by a 55 gallon drum and wheeled dolly.  C) The exit route located in the northwest corner of the "warehouse" area of the facility was obstructed by a seven foot tall wheeled staircase.

29 CFR  1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   On or about February 17, 2022, an employee performed servicing on machinery to include the Peerless Mixer (Model: HM3004W,Serial #204191). The employer had not established a program consisting of energy control procedures, employee training and inspections to ensure the startup or release of stored energy could not occur while servicing or maintenance.
