29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. (a) On or about February 15, 2022 the company failed to receive approval from Caterpillar to modify the forks on the lift and attach a boom to lift granite slabs with a weight of approximately 1200 pounds, exposing employees to a struck-by hazard.
