29 CFR  1910.95(i)(2):Employers shall ensure that hearing protectors are worn:  a) 214 Gates Rd Little Ferry, NJ: An employee using hammers and automatic wrenches to remove dried concrete from framing was monitored for 464 minutes and was exposed to continuous noise at 94.2 decibels averaged over 8-hours with zero exposure averaged for any unsampled time. The equivalent dose was 181% of OSHAs Permissible Exposure Limit for Noise, which is 90 decibels. The employer did not ensure that hearing protectors were worn, on or about February 17, 2022.  b) 214 Gates Rd Little Ferry, NJ: An employee using hammers and automatic wrenches to remove dried concrete from framing was monitored for 454 minutes and was exposed to continuous noise at 97.7 decibels averaged over 8-hours with zero exposure averaged for any unsampled time. The equivalent dose was 292% of OSHAs Permissible Exposure Limit for Noise, which is 90 decibels. The employer did not ensure that hearing protectors were worn, on or about February 17, 2022.

29 CFR  1910.95(k)(1):The employer shall train each employee who is exposed to noise at or above an 8-hour time weighted average of 85 decibels in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program.  a) 214 Gates Rd Little Ferry, NJ: An employee using hammers and automatic wrenches to remove dried concrete from framing was monitored for 464 minutes and was exposed to continuous noise at 94.2 decibels averaged over 8-hours with zero exposure averaged for any unsampled time. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.  a) 214 Gates Rd Little Ferry, NJ: Employees were exposed to overhead hazards while walking around the storage yard. The employer did not ensure concrete frame pieces did not create a hazard. Concrete frame pieces were stacked on frames that were not interlocked so that they were stable and secure against sliding or collapse, on or around February 17, 2022.

29 CFR  1910.252(c)(1)(ii): Screens. When welding must be performed in a space entirely screened on all sides, the screens shall be so arranged that no serious restriction of ventilation exists.
