29 CFR 1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part. a) Alfonso Vallejos, at a jobsite located at 7274 Quailview Drive, Lot 1257, Sunbury, Ohio, 43074: On and/or preceding February 16, 2022, the employer failed to initiate and maintain a safety and health program such as but not limited to a scaffolding safety program and fall protection program for employees conducting siding work.

29 CFR 1926.451(g)(4)(i):Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a) Alfonso Vallejos, at a jobsite located at 7274 Quailview Drive, Lot 1257, Sunbury, Ohio, 43074: On and/or preceding February 16, 2022, the employer failed to ensure employees were protected from falls while working from a Pump Jack Scaffold. An employee was working from the elevated platform of a Pump Jack Scaffold without a guardrail installed on the end of the platform, thereby being exposed to a fall hazard of 13 feet, 10 inches.
