29 CFR 1910.134(f)(2): Employees using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually: Johnson Matthey, Inc., located at 456 Devon Park Drive Wayne, PA: (a) On or about February 14, 2022, the employer did not ensure that employees required to wear full face elastomeric respirators were fit tested prior to require use. Employees such as electrical technician are required to wear full face elastomeric respirators such as but not limited to 3M 6800/6900 with multi gas/vapor cartridges to protect against toxic vapor while performing the ammonium hydroxide (30%) tote changeout. (b) On or about February 14, 2022, the employer did not ensure that employees required to wear full face elastomeric respirators were annually fit tested. Employees such as electrical technician are required to wear full face elastomeric respirators such as but not limited to 3M 6800/6900 with multi gas/vapor cartridges to protect against toxic vapor while performing the ammonium hydroxide (30%) tote changeout.

29 CFR 1910.134(e)(7)(ii): Additional medical evaluations that complied with the requirements of 29 CFR 1910.134 were not provided when a PLHCP, supervisor, or the respirator program administrator informed the employer that an employee needed to be reevaluated: (a) Johnson Matthey, Inc., located at 456 Devon Park Drive Wayne, PA: On or about February 14, 2022, the employer did not provide an additional medical evaluation when a physician provided a recommendation to follow up with the employee's primary care physician. The employee was required to wear hooded powered air-purifying respirators such as but not limited to 3M Breathe Easy Turbo Powered Air Purifying Respirator (PAPR) while performing the ammonium hydroxide (30%) tote changeout.
