29 CFR  1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:   a) Job site, 147 39th Street, Pittsburgh, PA: On or about February 15, 2022, during siding activities, the employer did not ensure that appropriate actions were taken to discover and abate hazardous conditions in the workplaces. Deficiencies noted during the inspection included, but were limited to, at times not using any fall protection where fall hazards of up to 4 stories existed, installing fall protection anchor points in the immediate vicinity of the edge of a roof and using an improperly built scaffold system where a piece of wood rested on the rung below the very top of a step ladder.
