29 CFR  1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  a) Worksite: Three 100-foot extension cords were  connected together to create a 300 foot cord. This violated the UL Listing by being daisy chained and of such length that voltage may move  too slow to trip the GFCI.
