1926.21(b)(2) The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Location - 411 Waverley Oaks Road, Waltham, MA. (right corner of building facing from road). On or about February 22, 2022, three employees were not protected from falling while engaged in steel erection activities while working/walking on an unprotected roof edge which was greater than twenty one feet above lower level.
