29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about February 12, 2022, and at times prior thereto: a. In the Bolt Department Area, employees were exposed to caught-by and struck-by hazards when operating and performing work on unguarded rotating parts, including rotating rods on the #3 Landis Threader. b. In the Bolt Department Area, employees were exposed to caught-by and struck-by hazards when operating and performing work on unguarded rotating parts, including rotating rods on the #1 Landis LO-1147 Threader. c. In the Bolt Department Area, employees were exposed to caught-by and struck-by hazards when operating and performing work on unguarded rotating parts, including rotating rods on the #2 Landis Maiden MB Series Threader. d. In the Bolt Department Area, employees were exposed to caught-by and struck-by hazards when operating and performing work on unguarded rotating parts, including rotating rods on the #4 Landis Threader. e. In the Bolt Department Area, employees were exposed to caught-by and struck-by hazards when operating and performing work on unguarded rotating parts, including rotating rods on the #5 Landis Maiden MB Series Threader. f. In the Bolt Department Area, employees were exposed to caught-by and struck-by hazards when operating and performing work on unguarded rotating parts, including rotating rods on the #6 Landis 10TRRB Threader. g. In the Bolt Department Area, employees were exposed to caught-by and struck-by hazards when operating and performing work on unguarded rotating parts, including rotating rods on the #7 Landis 10TRRB Threader. h. In the Bolt Department Area, employees were exposed to caught-by and struck-by hazards when operating and performing work on unguarded rotating parts, including rotating rods on the #10 Landis 16TRB Threader. i. In the Bolt Department Area, employees were exposed to caught-by and struck-by hazards when operating and performing work on unguarded rotating parts, including rotating rods on the#11 Landis 16TRB Threader. j.
