29 CFR 1960.66(b): Federal agency injury and illness recording and reporting shall comply with the requirements under 29 CFR 1904, subparts C, D, E, and G, except that the definition of "establishment" found in 29 CFR 1960.2(h) will remain applicable to Federal agencies. 29 CFR 1904.40(a) reference: Providing records to government representatives. Basic requirement. When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours. a) USGS Water Science Center, located at 2130 SW 5th Avenue, Portland Oregon, on February 23, 2022, the employer failed to provide a copy of the OSHA-300, OSHA-300A and OSHA-301 forms of the previous previous calendar year to the representative of the Occupational Safety and Health Agency within 4 hours of being requested.

29 CFR 1960.72(a): The agency failed to submit to the Secretary each year all information included on the agency's previous calendar year's occupational injury and illness recordkeeping forms, including the information entered on the OSHA Form 300, Log of Work-Related Injuries and Illnesses (or equivalent); OSHA Form 301, Injury and Illness Incident Report (or equivalent); and OSHA Form 300A, Summary of Work-Related Injuries and Illnesses (or equivalent). a) USGS Water Science Center, located at 2130 SW 5th Avenue, Portland Oregon, on February 23, 2022, the employer failed to report to the Occupational Safety and Health Agency the injury and illness information included on the agency's 300, 300A and 301 forms of the previous previous calendar year.
