29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about February 21, 2022, the employer did not ensure that periodic inspections of energy control procedures were conducted at least annually to identify and correct any deviations or inadequacies.  Employees who engaged in servicing and maintenance activities on the large leveler line were not familiar with the energy isolation steps needed to control hazardous energy sources, nor of any requirement to use them.  If the periodic inspections had been performed as required, these knowledge deficiencies could have been identified and corrected.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  On or about February 21, 2022, the employer did not ensure that employees who performed service and maintenance tasks on the large leveler machine were adequately trained on the means and methods needed to effectively  isolate and control hazardous energy sources.  Employees were exposed to the potential for unexpected start-up, energization and/or release of stored energy which could result in caught-on, crushing and amputation hazards while clearing jams on the large leveler line.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):  On or about February 21, 2022, the employer did not ensure that employees implemented energy control application steps prior to allowing them to enter the scrap conveyor cage and remove product that had not fed correctly into the stacker.  The machine was not shut down or turned off prior to employees engaging in this servicing and maintenance activity, exposing them to struck-by and crushing hazards from the unexpected movement of the machine.    As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
