OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards: a. On or about February 24, 2022, employees were exposed to struck-by and crushing hazard, in that the Crown brand PC4500-60 series lift trucks and pallet rack system's first level horizontal pallet shelf beams were not higher than the top of the forklift operator compartment.
