29 CFR  1910.140(d)(2)(ii): The employer did not ensure that personal fall arrest systems are rigged in such a manner that the employee cannot free fall more than 6 feet (1.8 m) or contact a lower level:   (a) Williams-Sonoma Stores, Inc. at 5503 Havana Street in Denver, Colorado: On or about February 24, 2022, and at times prior, employees were not protected from contacting a lower level when working at heights on a Raymond Order Picker 560-OP30TT. The personal fall arrest system did not provide adequate fall protection for the expected amount of clearance from the operator position to the lower level. This exposed employees to a fall hazard.

29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) Williams-Sonoma Stores, Inc. at 5503 Havana Street in Denver, Colorado: On or about February 24, 2022, and at times prior, the employer did not develop a lockout/tagout program with procedures, training and periodic inspections, to ensure employees and temporary workers were protected from the unexpected startup of equipment when they were required to clean cardboard shavings out of the wire slits on the Harris horizontal cardboard baler ram. This condition exposed the workers to crushing hazards.

This exposed employees and temporary workers to crushing hazards.  (c) Williams-Sonoma Stores, Inc. at 5503 Havana Street in Denver, Colorado: On or about February 24, 2022, and at times prior, the employer did not ensure that workers were protected from the point of operation of the Harris horizontal baler in the recycling area. This exposed employees and temporary workers to safety hazards related to impeding their ability to exit the building in an emergency.  (b) Williams-Sonoma Stores, Inc. at 5503 Havana Street in Denver, Colorado: On or about February 24, 2022, the emergency exit route in the inbound department on the south side of the building was obstructed by mattresses and boxes on pallets. This exposed employees and temporary workers to safety hazards related to impeding their ability to exit the building in an emergency.   Williams-Sonoma Stores, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1281710, Citation 1 Item 2, and was affirmed as a final order on January 10, 2018, with respect to a workplace located at 5145 Peachtree Parkway, Norcross, Georgia 30092.   Williams-Sonoma Stores, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1446085, Citation 1 Item 1B, and was affirmed as a final order on January 13, 2020, with respect to a workplace located at 3420 Urbancrest Industrial Drive, Grove City, Ohio 43123.
