OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to Struck-by/Caught between hazards associated with damage to the structural components of storage rack systems, storage rack anchor bolt deficiencies, and a lack of information conveyed to employees on the load limits associated with storage rack systems:  a)	DFA Dairy Brands Fluid, LLC-Dean Dairy, "Wendy's racking system-COR OA" on or about February 24th, 2022 and times prior thereto:    A total of 4 horizontal beams associated with the "Wendy's racking system COR OA" were damaged and completely unattached from the column uprights.  In addition, a total of 2 diagonal braces associated with the "Wendy's racking system COR OA" were damaged and completely unattached from the column uprights.    b)	DFA Dairy Brands Fluid, LLC-Dean Dairy, "Wendy's racking system-COR OA-Northwest corner of rack" on or about February 24th, 2022 and times prior thereto:  Two anchor bolts associated with the upright column on the northwest side of "Wendy's racking system COR OA" were damaged in that, they were bent and detached from the floor.  c)	DFA Dairy Brands Fluid, LLC-Dean Dairy, "Wendy's racking system-COR OA and COR OB" on or about February 24th, 2022 and times prior thereto: The load ratings for the  "Wendy' racking systems COR OA and COR OB" were not displayed and/or conveyed to employees.  d)	DFA Dairy Brands Fluid, LLC-Dean Dairy, "Wendy's racking system-COR OB" on or about February 24th, 2022 and times prior thereto:   One diagonal brace and horizontal beam associated with the "Wendy's racking system COR OB" was damaged and beginning to become detached from the column upright.
