29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about February 24, 2022, employees were exposed to a cave in while performing sewer line replacement at a commercial site located at 12920 Corporate Drive, Parma, Ohio 44130. An employee was working in a trench that was approximately 24 feet long, 13 feet wide, 4.5 feet deep on the north side and 6.5 feet on the south side. The employee was working near the south side and the employer did not ensure that he was using cave in protection.
