29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) PetDine, LLC, at 4500 Innovation Drive, Fort Collins, CO 80525: On and before February 15, 2022, the employer did not ensure that procedures were developed, documented, and utilized for the control of energy when employees used a hand-held paddle to dislodge product in the hopper that fed product into the Soft Chew Line extruder. The machine was energized and operating with this task was conducted. This condition exposed the employees to crushing and amputation hazards.
