The employer did not ensure the loft access area was protected by the use of guardrail systems or personal fall arrest systems. Employee(s) were exposed to a 9-foot fall hazard.

29 CFR  1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Pipe Shop: On and/or before February 24, 2022 employee(s) used the grinder/buffer to grind down parts and/or tools. The grinders black cord had damage to the jacket/insulation exposing the inner wires. Employee(s) were exposed to electrical shock and/or fire hazard(s).
