29 CFR 1910.28(b)(1)(i):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system: (a) Mix Plant: On or about February 24, 2022 and at times prior, employees were exposed to falls of approximately 19 ft. while accessing a ladder from the mix plant roof to perform maintenance on legs. The employees were not using fall arrest systems and the roof did not have a guardrail or safety net system. Note: Abatement certification is required for this item.

The pumps on the fuel tanks are powered via extension cords that are zip-tied to the building structure and plugged into a 3-plug outlet adapter.
