29 CFR  1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  140 Crystal St. Lenox, MA 01242 On 02/28/2022,  and at times prior, the employer had not performed triennial performance evaluations for all operators to ensure the operators have the knowledge and skills needed to operate the powered industrial trucks safely.

29 CFR 1910.178(l)(6): The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:  140 Crystal St. Lenox, MA: On February 28, 2022 and at times prior, the employer had no certification documents to demonstrate that all powered industrial truck operators had received their performance evaluations. Sixteen PIT operators were initially trained and allowed to operate PIT's, however, the employer did not have a certification that included the identity of the person who performed the training.
