29 CFR 1910.178(l)(6): Certification. The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. (a) Warehouse: On or prior to 2/22/22, the employer did not maintain a written certification that operators of forklifts including, but not limited to, the Toyota 8FGCU18, 8FBE18U, and 8FGCU25 counter-balance forklifts, were trained and evaluated in safe operation. Vanity Art was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(6), which was contained in OSHA inspection number 1532833, citation number 1, item number 6 and was affirmed as a final order on 7/16/21, with respect to a workplace located at 21 Simmons Lane, Menands, NY 12204.
