29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)Back of  Building/Exterior: Employees were exposed to head injury while installing siding without the use of protective helmets. Employees were using a pneumatic nail gun, on or about  2/24/22.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Back of the Building Exterior: Employees were exposed to eye/face injuries while installing siding with a pneumatic nail gun, without the use of eye and face protection. Employees were not protected with any type of eye/face protection, on or about 2/24/22.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  a) Back of the Building Exterior: Employees were exposed to a 14 foot fall hazard while engaged in siding activities from a ladder jack scaffold that was 24 feet long. The end of the platform was not designed and installed so that the cantilevered portion of the platform was able to support employees without tipping, the ladder jack scaffold had a 3 and 8 foot overhang over the support, on or about 2/24/22

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a) Back of the  Building Exterior: Employees were exposed to a 14 foot fall while engaged in siding work from a pump Jack  scaffold, without utilizing end rails or personal fall arrest system, on or about 2/24/22.  b) Building Exterior right side: Employees were exposed to a 14 foot fall while engaged in siding work from a ladder Jack  scaffold, without utilizing end rails or personal fall arrest system, on or about 2/24/22.
