29 CFR 1910.147(f)(1): In situations in which lockout or tagout devices had to be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the sequence of actions specified in subparagraphs (i)-(v) were not followed: a) At the FedEx facility, where specific procedures for troubleshooting and testing the NQIA conveyor belt were not developed and documented, exposing employees to amputation hazards.
