29 CFR 1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: Employees are exposed to struck-by and explosion hazards. Employees are exposed to struck-by and explosion hazards during powered industrial truck operations in that compressed gas cylinders are not being stored in-accordance with the Compressed Gas Association Pamphlet P-1-1965. The employer is storing both charged and empty compressed gas cylinders in a manner that combines expended and charged cylinders together with no signs or storage identification plans separating the two. Additionally, the compressed gas cylinders that are being stored in the storage rack are exposed to cuts, abrasion and subject to being struck by material handling equipment in that the compressed gas cylinders were being stored in a rack with an open, unprotected side.

1910.212(a)(3) Point of operation guard were not provided: Employees were exposed to amputation and struck-by hazards. Employees engaged in fabrication activities in the maintenance area were exposed to amputation and struck-by hazards in that employees were permitted to operate the DeWalt magnetic drill press, model DWE1622, serial # 020791, without point-of-operation guarding.
