On or about March 01, 2022, at the address of 2432 Hayes Ave. Fremont, OH 43420: The employer did not ensure that each powered industrial truck operator successfully completed the training and evaluation, specified by OSHA PIV training standards prior to permitting employees to operate powered industrial trucks. Failure to provide proper training and evaluation to PIV operators may lead to struck-by injuries to operators, as well as other employees working in and around forklift operating areas.

On or about January 26, 2022, at the address of 2432 Hayes Ave. Fremont, OH 43420: The employer failed to report on employee hospitalization (admittance) following a workplace incident (burns). The employer or employee representative failed to contact the Toledo Area Office or the 24 hour OSHA number to notify the agency of the employee hospitalization within 24 hours of admittance. The injured employee was admitted on January 25, 2022, for related in-patient treatment. The employer failed to contact OSHA between the time of admittance and the initiation of this inspection on March 1, 2022.
