29 CFR 1910.67(c)(2)(v):A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift. On or about March 1, 2022, an employee was exposed to a fall of approximately 15 feet while performing tree removal activities at a residential home located at 1585 Parkwood, Lakewood, Ohio 44107. The employee was working in the lift and the employer did not ensure that he was using fall protection.
