29 CFR 1910.134(f)(1):The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT): Facility at 305A Maple Street, East Longmeadow, MA: On or about February 24, 2022, the employer did not ensure that all employees required to wear N95 respirators had passed a qualitative or quantitative fit tests prior to use. An employee, who did not pass a fit test, continued to wear an N95 respirator that may not have provided the expected level of protection from respiratory hazards. Employees are required to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed COVID-19 positive residents.

29 CFR 1910.134(f)(2):The employer did not ensure that employees using a tight-fitting respirator were fit tested prior to initial use of the respirator, whenever a different respirator facepiece was used, and at least annually thereafter: Facility at 305A Maple Street, East Longmeadow, MA: On and before February 11, 2022, the employer did not provide a fit test to all employees required to wear a respirator, when a different respirator was used and at least annually thereafter. At least two employees, that were required to wear respirators, had not had initial fit tests when a different respirator was used and one employee had not had an annual fit test. Employees were required to wear N95 filtering-facepiece respirators to protect against SARS-CoV-2 virus while providing care, such care as feeding, washing, dressing, and other personal care, to suspected and confirmed COVID-19 positive residents. East Longmeadow Management Systems, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.134(f)(2), which was contained in OSHA Inspection number 1487375, citation number 1 item number 1d and was affirmed as a final order on December 3, 2022, with respect to a workplace located at 305A Maple Street, East Longmeadow, MA 01028.
