29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On March 1, 2022, during the installation of plywood sheathing on a newly constructed residential home located at 1953 Blossom Grove Dr. in Windsor, CO, the employer failed to ensure that employees were protected from a fall hazard by conventional fall protection and/or devices. Employees were exposed to falls from heights of 14 (+) feet, from the edge of the roof to the ground below.

29 CFR 1926.602(c)(1)(vii): Unauthorized persons were allowed to ride on a powered industrial truck: (a) On March 1, 2022, during the installation of plywood sheathing on a newly constructed residential property located at 1953 Blossom Grove Dr. in Windsor, CO, employees standing on a piece of plywood that was resting on forks of a JLG Model G10-55A powered industrial truck approximately 14 (+) above ground level were not protected from falling.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: (a) On March 1, 2022, during the installation of plywood sheathing on a newly constructed residential property located at 1953 Blossom Grove Dr. in Windsor, CO, the employer failed to ensure that employees were protected from a fall hazard by providing employees with a ladder that was free of structural defects. A Werner DI328-2 Type I ladder with a damaged rung was being used by employees for access/egress from the roof of the house. Employees were exposed to falls from heights of 14 (+) feet, from the ground to the edge of the roof.
