29 CFR  1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  a. Warehouse - On or about March 2, 2022,  employees were exposed to struck-by hazards, when required to operate  Toyota Electric Forklift #FL16, Serial No.  13731 . The power industrial vehicle did not have a functioning horn and was not removed from service.  b. Warehouse - On or about March 2, 2022,  employees were exposed to struck-by hazards, when required to operate Toyota Electric Forklift #FL34, Serial No. 27097. The power industrial vehicle did not have a functioning horn and was not removed from service.
