29 CFR 1926.451(g)(1)(i):Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system; On or about March 2, 2022, during the installation of roofing material on a two-story hotel, located at 30100 Clemens Road, Westlake, Ohio, the employer did not ensure employees working approximately 16 to 17 feet above the ground from a ladder jack scaffold were protected by a personal fall arrest system(s).

29 CFR 1926.501(b)(11):"Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about March 2, 2022, during the installation of roofing material on a two-story hotel located at 30100 Clemens Road, Westlake, Ohio, the employer did not ensure employees working from an approximately 7 in 12 slope roof were protected from a fall of approximately 17 feet (two-stories), by utilizing a guardrail system, safety net system and/or personal fall arrest system. Oscar Castillo, D.B.A. Storm Guard/Storm Guard Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1342300, citation number 1, item number 3 and was affirmed as a final order on November 23, 20218, with respect to a workplace located at 6000 Rockside Woods Blvd, Independence, Ohio, 44131.
