On or about March 1, 2022, on the northside of the site an employee was performing roofing work without the proper/inadequate fall protection on an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about March 1, 2022, employees were performing roofing work on the northeast side of the site using a ladder to gain access to the roof area that was not extended three feet above the landing.
