OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to chemical hazards. a) On or about 03/02/22, in the Fertilizer building located at 107 North Main Street, Bazine, KS: The eye wash station was not inspected weekly, monthly or annually. Among other feasible and acceptable means of abatement: To abate these hazards comply with the American National Standards Institute (ANSI) Standard Z358.1- 2014 Emergency Eyewash and Shower Equipment, as follows: a) Ensure the path of travel is free of obstructions that may inhibit its immediate use to comply with ANSI Z358.1 2014, Section 5.4.2, Installation. b) Ensure the eye wash station is visually checked weekly to determine if flushing fluid needs to be changed or supplemented. Such inspection shall be conducted in accordance with manufacturer s instructions, to comply with ANSI Z358.1 2014, Section 5.5.4, Maintenance and Training. c) Ensure the eye wash station is cleaned per manufacturers instructions.

29 CFR 1910.219(c)(4)(i): Unguarded shaft end(s) projected more than one half the diameter of the shaft: Employees were exposed to caught-in hazards. Employees engaged in boot pit operations while the main shaft is operating, were exposed to caught-in hazards in that the rotating shaft end was not guarded. The rotating shaft end is 32" from the floor to the exposed shaft end, additionally, the shaft end projected 2" from the surface with a shaft diameter of 2".
