29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition.  (a) On or about March 3, 2022, wood dust accumulation at the Baker Products Model A Horizontal Band Saw was not regularly cleaned up,  exposing employees to fire and combustion hazards, and inhalation of toxic chemical hazards.   Exterior surfaces of the saw,  and the work areas around the saw were covered with an adequate layer of wood dust to facilitate ignition when sparks are created as a result of metal on metal contact that occurs when the saw cuts through metal such as nails and staples, or can also result from the heated blade motor that was covered in wood dust.

29 CFR  1910.176(c): Storage areas were not kept free from accumulation of materials that constituted hazards from tripping, fire, explosion or pest harborage:  (a) On or about March  3, 2022, in the main area of the facility and near  the Baker Products Model A Horizontal band saw which is used to cut raw material and used pallets for recycling purposes, fine wood dust was allowed to accumulate in quantities to support combustion. This exposed employees to fire, combustion and explosion hazards.

(b) Machine #3 -  Large Cross cut (chop) saw: On or about March 3, 2022, the employer did not develop, document, and maintain procedures for the control of potentially hazardous energy for employees  engaged in maintenance activities.  (c)  Machine #4 - Small Cross cut (chop) saw: On or about March 3, 2022, the employer did not develop, document,  and maintain procedures for the control of potentially hazardous energy for employees  engaged in maintenance activities.  (d) Machine #5 - Baker Products Horizontal band saw: On or about March 3, 2022, the employer did not develop, document,  and maintain procedures for the control of potentially hazardous energy for employees  engaged in maintenance activities.  (e) Machine #6 -  Air compressor: On or about March 13, 2022, the employer did not develop, document,  and maintain procedures for the control of potentially hazardous energy for employees  engaged in maintenance activities.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   (a) On or about March 3, 2022, the employer did not develop or implement a written hazard communication program in accordance with 29 CFR 1910.1200(d)(1) that would describe or include at least the following:  1) Requirement for labeling of containers of hazardous chemicals;  2) Safety Data Sheet (SDS) availability;  3) Training of employees;  4) Inventory listing of hazardous chemicals known to be in the workplace;  5) Methods to inform employees of the hazards of non-routine tasks; and  6) Methods to inform other employee of safety data sheet availability; the labeling system; and any precautionary measures to protect employees.  Workers were exposed to hazardous chemicals, including wood dust, nail gun lubricant, and propane.
