On or about February 24th, 2022, an employee of Kendall Patient Recovery U.S. LLC located at 222 Sherwood Rd., Norfolk, NE 68701, was engaged in operating an ACRA: NM-5SAC Milling Machine, Serial Number 001052 without a guard resulting in an amputation.
