29 CFR  1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.  a. Located at 1150 Kinear Road in Columbus, Ohio: On or about March 3, 2022, two employees were installing an approximately twenty-foot section of gutter from the roof of a four-story apartment building with personal fall arrest systems attached to a single roof anchor that was existing on the building. The Super Anchor 2000 roof anchor was only rated for one employee attached, thereby exposing employees to a fall hazard from approximately forty feet.
