Linfoot Street Wauseon, Ohio, the employer did not ensure employees were protected from entering the point of operation while operating machine #252, Havir Manufacturing Co. 1 1/2 ton full revolution mechanical punch press.  An employee received amputation injuries to the first and second fingers of the left hand while operating machine #252.

29 CFR  1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment:  a)  Specialized Inspection Group located at 4566 County Road 19 Pettisville, OH 43553: On or about March 21, 2022, at 135 E. Linfoot Street, Wauseon, OH, the employer did not establish and follow a program of periodic or regular inspections for press Machine #252 (Havir Manufacturing Co. 1 1/2 full revolution punch press machine) to ensure that all parts, including the overhead restraint device were in a safe operating condition.  Employees tasked to operate machine #252 were exposed to crush and point of operation hazards from press deficiencies in that the overhead restraint device had been removed and not properly installed.
