29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a. On or about January 22, 2022, in the boiler room, employees were exposed to a fall hazard exceeding 4 feet while working on the boiler #1 platform which did not have a fall protection system in place.  b. On or about February 26, 2022, in boiler room, employees were exposed to a fall hazard of more than 4 feet while looking at a valve leak on top of boiler #2 and a section of the boiler platform did not have a fall protection system in place.

29 CFR  1910.29(b)(2): The employer did not ensure that guardrail systems met the requirement that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members that are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high.  On or about January 22, 2022, in the boiler room, employees were exposed to a fall hazard exceeding four feet while working on the boiler 1 and 2 platforms which did not have a midrail or other member between the working surface and the top edge of the guardrail.

On or about March 2, 2022, in the DG-80 area, an employee was exposed to a struck-by hazard while inspecting the dies on the hammer without utilizing procedures for controlling hazardous energy.  b. On or about March 2, 2022, in the rough machine shop, an employee was exposed to struck-by hazards while performing maintenance and servicing work on Toyoda#2 without utilizing procedures for controlling hazardous energy.
