On or about January 22, 2022, in the boiler room, employees were exposed to a fall hazard exceeding 4 feet while working on the boiler #1 platform which did not have a fall protection system in place. b. On or about February 26, 2022, in boiler room, employees were exposed to a fall hazard of more than 4 feet while looking at a valve leak on top of boiler #2 and a section of the boiler platform did not have a fall protection system in place.
