29 CFR  1910.157(e)(3): The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check.   a) Facility - On or about March 3, 2022, portable fire extinguishers were observed as having been last inspected in 2020 and had not been inspected annually in accordance with this section.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.1030(c)(1)(ii):The employer's Exposure Control Plan did not contain at least the elements in 29 CFR 1910.1030(c)(1)(ii)(A) through (c)(1)(ii)(C):  a) Facility - On or about March 3, 2022, employees were exposed to bloodborne pathogens and the employer did not develop and implement a written exposure control plan with the required elements in accordance with this section.  Dr. Mel A. Burchman dba Langhorne Family Smiles was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1030(c)(1)(ii), which was contained in OSHA inspection number 1511105, citation number 1, item number 1 and was affirmed as a final order on June 1, 2021, with respect to a workplace located at 712A Trenton Rd. Langhorne, PA 19047.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.
