29 CFR 1910.23(c)(11): The employer did not ensure that portable ladders used to gain access to an upper landing surface that have side rails that extend at least 3 feet (0.9 m) above the upper landing surface. a) Air Compressor Room: On or about February 23, 2022, and times prior an employee used an 8 foot step ladder to gain access to the roof of the air compressor room to blow out the filter. The employee is exposed to a fall hazard of 8 feet roof to floor while using the ladder to climb onto or off of the roof. b) Dryer Room: On or about February 23, 2022, and times prior an employee used an 8 foot ladder to gain access to the roof of the dryer room to troubleshoot a problem on the furnace. The employee is exposed to a fall hazard of 8 feet roof to floor while using the ladder to climb onto or off of the roof.

The roof is approximately 8 feet above ground and the employee was not provided with a fall protection system such as guard rails or personal fall arrest. b) Kiln: On or about February 23, 2022, and times prior, an employee stood on a platform at the top edge of the kiln to grease a pulley. The kiln top is approximately 6 feet above ground and the employee was not provided with a fall protection system such as guard rails or personal fall arrest.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Jet Drill Press: On or about November 2, 2021, and times prior, an employee used a Jet drill press to drill holes in product realization. The rotating drill bit was not equipped with a guard to prevent employee exposure to flying chips and rotating parts. b) Finishing Area Pour cup Machine: On or about March 2, 2022, and times prior, employees used a pour cup machine to knock off burrs on products. The machine is equipped with a guard however the guard is not used on all products to prevent employee exposure to rotating parts.
